In consideration of your accepting this entry thereby for myself, my heirs, executors and administrator
waiver and release any and all right and claim for damages I may have against the City of Parma, Parma City Schools, Parma Hospital Foundation, Hermes Sports & Events, Parmatown One, LLC and the event sponsors, their representatives, successors, and assigns from any and all injuries suffered by me in said event. I also give my permission for the free use of my name and/or picture in any broadcast, telecast or other account of this event. I further attest and certify that I am physically fit, and have sufficiently trained for competition in this event, and my physical condition has been verified by a medical doctor.
